ABSTRACT

Frailty is a nonspecific state with the decline of physiological reserve leading to the increased vulnerability and decreased anti-stress ability in the elderly. Frailty can cause cognitive impairment and physical function decline, and increase the risk of a series of adverse events such as hospitalization, disability and death in the elderly. Studies have shown that early intervention can reverse debilitating condition; therefore, early and timely intervention of frailty has important practical significance for improving the health status of the elderly and reducing the pressure on the medical system. This article reviews frailty interventions and their effects in the elderly, including sports intervention, nutrition intervention, drug intervention and integrated intervention, focusing on the research progress of frailty intervention, to provide reference for clinical management of frailty in the community-dwelling elderly.

ABSTRACT

Good sleep is essential for children′s physical and mental health, growth, and development.Adequate and high-quality sleep positively impacts children′s quality of life, memory, learning, attention, mood, and behavior.Children of different ages have different sleep needs.Children have various sleep problems in different cultures.Therefore, it is significant to guide children to get healthy sleep by popularizing the sleep problems of Chinese children of all ages and in all dimensions.Based on China′s social and cultural background, this paper summarizes the contents related to children′s sleep physiology, good sleep habits, common sleep problems, sleep apnea disorders, hypnagogic sleep, narcolepsy, insomnia, and other issues that interfere with children′s sleep.Chinese sleep experts summarize and interpret the 100 sleep health problems of children that medical workers and parents are most concerned about.To promote children′s sleep health and popularize solutions to sleep problems.

ABSTRACT

Objective: To investigate the influence of sleep fragmentation in infancy and toddler period on emotional and behavioral problems at the age of 6 years. Methods: Using a prospective cohort design, 262 children were extracted from mother-child birth cohort recruited from May 2012 to July 2013 in Renji Hospital, School of Medicine, Shanghai Jiao Tong University. Children's sleep and physical activities were assessed using actigraphy at 6, 12, 18, 24, and 36 months of age, from which the sleep fragmentation index (FI) at each follow-up point was calculated. Children's emotional and behavioral problems at 6 years of age were assessed using the strengths and difficulties questionnaire. Group-based trajectory model was applied to determine sleep FI in infancy and toddler period trajectory groups with Bayesian information criteria being used to determine the best fitting model. Children's emotional and behavioral problems between groups were examined with independent t test and linear regression models, etc. Results: A total of 177 children, with 91 boys and 86 girls, were included in the final analysis and were divided into 2 groups: high FI group (n=30) and low FI group (n=147). Compared with children in the low FI group, those in the high FI group presents with higher total difficulties score and higher hyperactivity or inattention score ((11.0±4.9) vs. (8.9±4.1), (4.9±2.7) vs. (3.7±2.3) scores, t=2.17, 2.23, both P<0.05, respectively), with the differences remaining significant after adjusting for covariates (t=2.08, 2.09, both P<0.05 respectively). Conclusion: High sleep fragmentation in infancy and toddler period is associated with more emotional and behavioral problems, especially hyperactivity or inattention problems, at 6 years of age.

ABSTRACT

OBJECTIVE@#To investigate the effects of gene of phosphate and tension homology (PTEN)-induced putative kinase 1 (PINK1)/Parkin pathway on hippocampal mitophagy and cognitive function in mice with sepsis-associated encephalopathy (SAE) and its possible mechanism.@*METHODS@#A total of 80 male C57BL/6J mice were randomly divided into Sham group, cecal ligation puncture (CLP) group, PINK1 plasmid transfection pretreatment groups (p-PINK1+Sham group, p-PINK1+CLP group), empty vector plasmid transfection control group (p-vector+CLP group), with 16 mice in each group. The mice in CLP groups were treated with CLP to reproduce SAE models. The mice in the Sham groups were performed laparotomy only. Animals in the p-PINK1+Sham and p-PINK1+CLP groups were transfected with PINK1 plasmid through the lateral ventricle at 24 hours before surgery, while mice in the p-vector+CLP group were transfected with the empty plasmid. Morris water maze experiment was performed 7 days after CLP. The hippocampal tissues were collected, the pathological changes were observed under a light microscope after hematoxylin-eosin (HE) staining, and the mitochondrial autophagy was observed under a transmission electron microscopy after uranyl acetate and lead citrate staining. The expressions of PINK1, Parkin, Beclin1, interleukins (IL-6, IL-1β) and microtubule-associated protein 1 light chain 3 (LC3) were detected by Western blotting.@*RESULTS@#Compared with the Sham group, CLP group mice in Morris water maze experiment had longer escape latency, shorter target quadrant residence time, and fewer times of crossing the platform at 1-4 days. Under the light microscope, the hippocampal structure of the mouse was injured, the neuronal cells were arranged in disorder, and the nuclei were pyknotic. Under the electron microscope, the mitochondria appeared swollen, round, and wrapped by bilayer or multilayer membrane structures. Compared with the Sham group, CLP group had higher expressions of PINK1, Parkin, Beclin1, LC3II/LC3I ratio, IL-6 and IL-1β in hippocampus, indicating that sepsis induced by CLP could activated inflammatory response and caused PINK1/Parkin-mediated mitophagy. Compared with the CLP group, p-PINK1+CLP group had shorter escape latencies, spent more time in the target quadrant and had more number of crossings in the target quadrant at 1-4 days. Under the light microscope, the hippocampal structures of mice was destroyed, the neurons were arranged disorderly, and the nuclei were pyknotic. Under transmission electron microscope, swollen and rounded mitochondria and mitochondrial structure wrapped by double membrane or multilayer membrane structure were observed. Compared with the CLP group, the levels of PINK1, Parkin, Beclin1 and LC3II/LC3 ratio in the p-PINK1+CLP group were significantly increased [PINK1 protein (PINK1/β-actin): 1.95±0.17 vs. 1.74±0.15, Parkin protein (Parkin/β-actin): 2.06±0.11 vs. 1.78±0.12, Beclin1 protein (Beclin1/β-actin): 2.11±0.12 vs. 1.67±0.10, LC3II/LC3I ratio: 3.63±0.12 vs. 2.27±0.10, all P < 0.05], while the levels of IL-6 and IL-1β were significantly decreased [IL-6 protein (IL-6/β-actin): 1.69±0.09 vs. 2.00±0.11, IL-1β protein (IL-1β/β-actin): 1.11±0.12 vs. 1.65±0.12, both P < 0.05], suggesting that overexpression of PINK1 protein could further activate mitophagy and reduce the inflammatory response caused by sepsis. There was no statistically significant difference in the above pathological changes and related indicators between Sham group and p-PINK1+Sham group, CLP group and p-vector+CLP group.@*CONCLUSIONS@#PINK1 overexpression can further activate CLP-induced mitophagy by upregulating Parkin, thereby inhibiting inflammation response and alleviate cognitive function impairment in SAE mice.

ABSTRACT

@#Objective To demonstrate the changes in flexor digitorum and extensor digitorum tension in the affected hands with shear-wave elastography (SWE) before and after manual digitorum sensory stimulation (MDSS) in hemiplegic patients with stroke. Methods A total of 51 hemiplegic post-stroke inpatients in the Department of Rehabilitation Medicine in Second Hospital of Anhui Medical University from April to June, 2020, underwent MDSS completed by a researcher who used a bare thumb and index finger to squeeze each nail bed as well as the sides of each fingertip in the affected hand. The stimulation intensity was the minimum that could cause finger extension without obvious pain, and the interval between two stimulations was five to ten seconds. Muscular tension of the flexor digitorum superficialis, flexor digitorum profundus, flexor pollicis longus and extensor digitorum were assessed with modified Ashworth Scale (MAS) and shear-wave velocity (SWV) of SWE on the affected side before and immediately after MDSS. MAS score was -1 as low muscular tension. Results The MAS scores of all the muscles significantly reduced after MDSS (|Z| > 2.843, P < 0.001); while the changes of SWV were not significantly in all the muscles with initially MAS score of 0 or -1 (t < 1.052, P > 0.05), and it reduced in those muscles with initial MAS scores of one to three (t > 2.672, P < 0.05). The SWV were positively correlated with the MAS scores both before and after MDSS (r > 0.334, P < 0.05). Conclusion MDSS can effectively, immediately, and safely relieves muscle spasms of the flexor digitorum and facilitate active finger extension in the affected hand for hemiplegic patients with stroke. SWE is useful for quantitatively and objectively evaluating muscular tension in the affected hand for hemiplegic patients with stroke.

ABSTRACT

Objective:To investigate the awareness and application of exercise prescription among general practice faculty in Taiyuan City, and to analyze the related factors.Methods:A questionnaire survey on the awareness and application of exercise prescriptions was conducted from July to September 2021 among general practice faculty in Taiyuan City selected by stratified random sampling method. The influencing factors of awareness and application were analyzed with logistic regression.Results:A total of 589 general practice faculty were enrolled in the study, 342 of whom were females (58.06%). The overall awareness rate of exercise prescription was 38.74% (1 369/3 534); the awareness rate of basic knowledge was 56.08% (991/1 767) and the awareness rate of core knowledge was 21.39% (378/1 767). Logistic analysis showed that specialty as general practitioner ( OR=1.66, 95% CI:1.07-2.57, P=0.024), experience of exercise prescriptions ( OR=2.07, 95% CI:1.37-3.12, P=0.001) and willingness of public promotion of exercise benefits ( OR=2.08, 95% CI:1.05-4.15, P=0.037) were factors related to higher awareness of exercise prescription. The survey showed that 231 responders (39.22%) had given patients exercise guidance, among whom 125 (54.11%) gave oral and specific practice guidance, 206 (89.18%) would ask patients to perform health screening before exercise, and 184 (79.65%) performed health fitness evaluation. The knowledge used for guidance was mostly from textbooks or related guidelines (42.42%, n=98), but none of them had prescribed standardized exercise prescriptions. Conclusions:The survey shows that the knowledge and application rate is relatively low among general practice faculty in Taiyuan City. It is necessary to strengthen the training of exercise prescription for them.

Resistance exercise plays an important role in management of cardiovascular disease and blood sugar control;and in improvement of skeletal muscle health and old-age debility. The purpose of this review is to summarize the preventive and therapeutic effects of resistance exercise on various chronic diseases, to improve the knowledge and awareness of the general practitioners for comprehensive understanding of the role of resistance exercise and to provide exercise guidance for patients with chronic diseases.

ABSTRACT

Objective:To apply multimodal ultrasound imaging in spasticity assessment for stroke patients with hemiplegia. Methods:From March to September, 2019, 44 inpatients with lower limb spastic hemiplegia after stroke (patients) and 46 healthy volunteers (controls) were scanned with two-dimensional ultrasound imaging, shear wave elastography and super microvascular imaging at the same area of gastrocnemius muscle of both sides of patients and right side of controls, in resting and maximum isometric contraction. The parameters of shear wave velocity (SWV), blood flow signal, pinnation angle (PA), fascicle length (FL) and muscle thickness (MT) were recorded. Results:SWV increased in the affected side of the patients compared with those of the unaffected side and controls in resting (t > 6.346, P < 0.01), while FL shortened (|t| > 6.235, P < 0.01), MT and PA changed compared with those of the unaffected side (|t| > 2.198, P < 0.05), and blood flow signal decreased compared with those of the controls (t = 2.604, P < 0.05). In maximum isometric contraction, the decrease of FL was less compared with those of the unaffected side and controls (Z > 6.703, P < 0.001). Conclusion:Ultrasound imaging can quantitatively evaluate spasticity of gastrocnemius muscle for patients with stroke in terms of morphological structure, blood flow, and muscle stiffness.

ABSTRACT

BACKGROUND@#Norepinephrine infusion decreases hypotension after spinal anesthesia during cesarean section. This study aimed to compare the efficacy of norepinephrine infusion and ephedrine bolus against post-spinal hypotension in parturients.@*METHODS@#In this double-blinded, randomized controlled clinical trial, parturients scheduled for elective cesarean section were randomly allocated to receive norepinephrine infusion (0.05 μg·kg-1·min-1) just before spinal anesthesia continuing for 30 min or ephedrine bolus (0.15 mg/kg) just before spinal anesthesia. A rescue bolus (5 μg norepinephrine for the norepinephrine group, and 5 mg ephedrine for the ephedrine group) was administered whenever hypotension occurred. Our primary outcome was the incidence of hypotension within 30 min of spinal anesthesia administration. Secondary outcomes included maternal and neonatal outcomes 30 min after spinal block, and neonatal cerebral oxygenation 10 min after birth.@*RESULTS@#In total, 190 patients were enrolled; of these patients, 177 were included in the final analysis. Fewer patients suffered hypotension in the norepinephrine group than in the ephedrine group (29.5% vs. 44.9%, odds ratio [OR]: 0.51, 95% confidence interval [CI]: 0.28-0.95, P = 0.034). Moreover, the tachycardia frequency was lower in the norepinephrine group than in the ephedrine group (OR: 0.22, 95% CI: 0.11-0.44, P < 0.001), and patients suffered less nausea and vomiting (OR: 0.28, 95% CI: 0.11-0.70, P = 0.004). There was no difference in Apgar scores and umbilical arterial blood gas analysis between the two groups. However, neonatal cerebral regional saturations were significantly higher after birth in the norepinephrine group than in the ephedrine group (mean difference: 2.0%, 95% CI: 0.55%-3.45%, P = 0.008).@*CONCLUSION@#In patients undergoing elective cesarean section with spinal anesthesia, norepinephrine infusion compared to ephedrine bolus resulted in less hypotension and tachycardia, and exhibited potential neonatal benefits.@*TRIAL REGISTRATION@#ClinicalTrials.gov, NCT02542748; https://clinicaltrials.gov/ct2/show/record/NCT02542748.

ABSTRACT

Objective:To understand the components, performance, acceptance test and quality assurance (QA) protocols of the new Varian Halcyon accelerator through the procedures of installation product acceptance (IPA).Methods:Per Varian IPA protocol, TG-142 for LINAC and TG-148 for tomotherapy QA protocols, the software license, safety interlock, mechanical accuracy, dosimetric performance and imager system were checked thoroughly. Some parameters were benchmarked to the conventional TrueBeam system.Results:The system has been fully licensed. Safety interlock was normal. Mechanical accuracy: The maximum deviation of beam stability per gantry rotation was 1.13%. The size of isocenter was>0.59mm. The offsets of MV imager, collimation rotation and absolute gantry rotation were 0.09mm, -0.21° and 0.11°, respectively. The maximum offsets of couch, virtual-to-isocenter were 0.15mm (vertical) and -0.04mm (vertical), respectively. Beam performance: The depth deviation of maximum dose was 0.1cm. The offset of percentage depth dose at 10cm was 0.5%. The maximum deviations of off-axis-intensity, symmetry, and repeatability were 0.9%, 0.94% and -0.44%, respectively. MV imager: The dark field mean pixel value, noise, corrected pixels, defective lines, sensitivity and linearity disparity of dose were 614, 4.4, 3626, 0, 19177, and 0.47%, respectively. All values were within the range of tolerance. Visual check of contrast resolution and small object detection was all satisfactory.Conclusions:Without Halcyon-specific TG report or guidelines, manufacture-provided IPA manual can be helpful with the installation of acceptance and QA protocols. IPA has been successfully performed for Halcyon at Peking University Cancer Hospital. The automated workflow improves the clinical efficiency by simplifying the operations.

ABSTRACT

Objective:To investigate the performance of W2 plastic scintillator in megavolt photon and electron beams.Methods:The photon and electron beam energy provided by linear accelerator was used to collect data of the W2 scintillator. The parameters include the electrometer reading stability, W2 dose and dose rate linearity, and angular response. And the dose uncertainty of the W2 correction factors was also investigated.Results:The standard deviation of the electrometer reading stability was between 0.03 and 0.47. The linear regression factors of W2 dose were all 1.0; the maximum deviation of the dose rates was 0.61%. The Cerenkov light radiation correction factor(CLR) for 6 and 10 MV were 0.741 and 0.746, respectively, and the CLR for 6, 9, 12 and 15 MeV were 0.750, 0.753, 0.757 and 0.757, respectively. The maximum deviation of dose uncertainty for 15 MeV was 3.15%.Conclusions:The signal obtained by the blue and green channel was no angular dependence, the same as the high-energy electron beam, which verified that the Cerenkov radiation correction factor has good linearity. W2 plastic scintillator can be applied to non-coplanar radiotherapy dosimetry.

ABSTRACT

Objective:To compare the application of continuous negative pressure wound therapy and conventional packing and pressure bandage in medium thick skin transplantation of difficult to fix wounds.Methods:96 patients who underwent medium thick skin transplantation of difficult to fix wounds in Suizhou Hospital Affiliated to Hubei Medical College from March 2019 to March 2020 were divided into two groups according to whether the patients were treated with continuous negative pressure wound therapy. 48 patients in the control group were treated with conventional packaging and pressure bandage, and 48 patients in the observation group were treated with continuous negative pressure wound therapy. The levels of white blood cell (WBC), C-reactive protein (CRP), skin graft survival area, skin graft survival ratio, skin hematoma ratio, length of hospital stay and incidence of complications were compared between the two groups.Results:The levels of WBC and CRP in the two groups at 7 and 14 days after treatment were significantly lower than those before treatment ( P<0.05); The levels of WBC and CRP in the observation group were significantly lower than those in the control group at 7 and 14 days after treatment ( P<0.05); The survival area and survival ratio of skin graft in the observation group were significantly higher than those in the control group ( P<0.05); The area and ratio of skin hematoma in the observation group were significantly lower than those in the control group ( P<0.05); The hospital stay in the observation group was significantly shorter than that in the control group ( t=10.472, P=0.001); The incidence of skin graft complications in the observation group was significantly lower than that in the control group (2.08% vs 12.50%) (χ 2=10.174, P=0.015). Conclusions:Continuous negative pressure wound in medium thickness skin transplantation of difficult to fix wound has obvious advantages in improving the survival area of skin graft, reducing skin graft complications and shortening hospital stay, which is worthy of clinical recommendation.

ABSTRACT

Objective:To assess the impact of integrated curriculum (vertically integrated curriculum system) and traditional curriculum system on the perceptions of post competency of undergraduate medical students.Methods:Bibliographic retrieval method and expert consultation were used to compile the questionnaire of perceptions on the post competency of undergraduate medical students. The questionnaire was conducted online to survey the undergraduate medical students from Shanghai Jiao Tong University School of Medicine during March to April, 2019. SPSS 20.0 software was applied for data analysis, chi-square test was used to analyze the classroom teaching forms of medical undergraduates who received different curriculum systems, and their cognition of the doctor post competency and cognitive pathways, and finally the top 10 important competencies were compared.Results:A total of 200 questionnaires were distributed and 167 were recovered, with a recovery rate of 83.5%. Workshop learning was more common in the vertically integrated undergraduate curriculum. Compared with Chinese traditional curriculum, the medical students taught by the vertically integrated undergraduate curriculum were much better informed about competency ( P<0.05). The access to knowledge of competency was also statistically different between two groups of students ( P<0.05). Conclusion:The vertically integrated curriculum is more efficient in helping undergraduate medical students to acquire knowledge about competency.

ABSTRACT

Objective:To investigate the clinical features of Hashimoto′s thyroiditis(HT) with elevated serum IgG4 levels and to guide the clinical practice.Methods:Serum IgG4 concentrations were detected in patients with HT by nephelometric immunoassay. These patients were classified into two groups according to the IgG4 levels: positive group(IgG4≥1.35 g/L), and negative group(IgG4<1.35 g/L). The clinical characteristics of these two groups, including age, gender, medical history, serological features, and ultrasound were compared.Results:Nine out of 111 patients with HT were positive with IgG4(8.11%), all of them were women. There was no significant difference in gender, age, BMI, dose of levothyroxine administration, FT 3, FT 4, and TSH levels between the two groups. The volume of thyroid in positive group was larger than that in negative group [57.81(38.36, 74.93) ml vs 25.07(18.48, 42.14) ml, P=0.015], and the level of thyroglobulin antibodies(TgAb; P=0.011) and thyroid peroxidase antibodies(TPOAb; P=0.025) in positive group were also significantly higher than those in negative group. Moreover, the positive group had a slightly higher risk of PTC than that of negative group(11.1% vs 2.94%, P=0.290), though the difference was not significant. Correlation analysis showed that IgG4 was positively associated with TgAb, TPOAb, and thyroid volume. Conclusions:The levels of TgAb and TPOAb are higher, the volume of thyroid gland is greater, and the risk of PTC with lymph node metastasis is more frequent in HT patients with elevated serum IgG4. For HT patient with elevated serum IgG4, their thyroid function and morphology should be more closely monitored.

OBJECTIVE:To observe the safety and other related indexes of canagliflozin in the treatment of type 2 diabetes complicated with high risk of cardiovascular disease. METHODS :Totally 306 patients,admitted to Hainan Provincial People ’s Hospital and Haikou People ’s Hospital ,with type 2 diabetes complicated with high risk of cardiovascular disease were selected from Dec. 2018 to Apr. 2019. They were divided into observation group (153 cases)and control group (153 cases)according to random number table . The control group was treated with in sulin,metformin or sulfonylureas conventional hypoglycemic therapy , and the observation group was treated with Canagliflozin tablets 100 mg,once a day ,po,on the basis of control group. The course of treatment was 1 year in both groups. The levels of HbA 1c,BMI,SBP,DBP and eGFR before and after treatment were observed in 2 groups,and the incidence of safety (including death from cardiovascular causes ,myocardial infarction ,ischemic stroke , hospitalization for heart failure and death from any cause etc. ) after treatment and serious ADR/ADE (including hypogly- cemia,diabetic ketoacidosis ,fracture,acute kidney injury 68622942。E-mail:zhaixin0123@126.com etc.)during the treatment were recorded. RESULTS :A total of 5 patients in the control group were not followed up , in which 3 quited and 2 were lost ;and 4 patients in the observation group were not followed up ,in which 1 quited and 3 were lost . Before treatment ,there were no statistical significance in the levels of HbA 1c,BMI,SBP,DBP and eGFR between 2 groups(P>0.05). After treatment ,HbA1c levels of 2 groups,BMI and SBP of observation group were all significantly lower than those before treatment with the same group ;HbA1c level and SBP of observation group were significantly lower than those of control group (P<0.05). eGFR levels of 2 groups after treatment were significantly higher than before treatment with the same group ,while the observation group was significantly higher than that of contrl group. The incidence of death from cardiovascular causes and death from any cause in observation group were significantly lower than control group (P<0.05). There were no statistically significant differences in other safety indexes and the incidence of serious ADR/ADE between 2 groups(P>0.05). CONCLUSIONS :Canagliflozin can significantly reduce the incidence of death from cardiovascular causes and death from any cause in type 2 diabetes patients complicated with high risk of cardiovascular disease,ameliorate blood glucose and blood pressure ,and do not increase the occurrence of serious ADR/ADE.

ABSTRACT

Objective@#To investigate the clinical features of Hashimoto′s thyroiditis(HT) with elevated serum IgG4 levels and to guide the clinical practice.@*Methods@#Serum IgG4 concentrations were detected in patients with HT by nephelometric immunoassay. These patients were classified into two groups according to the IgG4 levels: positive group(IgG4≥1.35 g/L), and negative group(IgG4<1.35 g/L). The clinical characteristics of these two groups, including age, gender, medical history, serological features, and ultrasound were compared.@*Results@#Nine out of 111 patients with HT were positive with IgG4(8.11%), all of them were women. There was no significant difference in gender, age, BMI, dose of levothyroxine administration, FT3, FT4, and TSH levels between the two groups. The volume of thyroid in positive group was larger than that in negative group [57.81(38.36, 74.93) ml vs 25.07(18.48, 42.14) ml, P=0.015], and the level of thyroglobulin antibodies(TgAb; P=0.011) and thyroid peroxidase antibodies(TPOAb; P=0.025) in positive group were also significantly higher than those in negative group. Moreover, the positive group had a slightly higher risk of PTC than that of negative group(11.1% vs 2.94%, P=0.290), though the difference was not significant. Correlation analysis showed that IgG4 was positively associated with TgAb, TPOAb, and thyroid volume.@*Conclusions@#The levels of TgAb and TPOAb are higher, the volume of thyroid gland is greater, and the risk of PTC with lymph node metastasis is more frequent in HT patients with elevated serum IgG4. For HT patient with elevated serum IgG4, their thyroid function and morphology should be more closely monitored.

ABSTRACT

Objective To compare two pretreatment plan QA methods for HalcyonTM accelerator using Portal Dosimetry (PD),and PTW OCTAVIUS 1500 detector array + Octagonal phantom (Oct 1500)respectively.Methods Parallel measurement-based pretreatment QA using two methods was performed for 22 IMRT/VMAT plans (74 fields) that have been used to treat 20 patients recruited in the Halcyon clinical trial.Several γ 2D comparisons were also applied to provide guidelines for Halcyon planning QA.Results Using Oct1500 method,the γ 2D passing rates for 74 fields in 22 Plans were 95.26±3.59,95.01±3.62 (Local Dose),99.05± 1.35,98.57± 1.96 (Max Dose) respectively.Two-related samples non-parametric tests suggested that the differences between the evaluation criteria were of statistical significance (Z =-7.220,-4.108,P<0.05).For PD method,the γ 2D passing rates were 84.11% ± 1.35% (1 mm/1%),99.07%± 1.35% (2 mm/2%),and 99.86% ± 1.35% (3 mm/3%).Two-related samples non-parametric tests suggested that the differences between evaluation criteria of PD method were statistically significant (Z =-7.475,-7.475,-6.906,P<0.05).For 74 fields and max dose,3 mm/3% evaluation criteria,two-related samples non-parametric tests suggested that the differences between PD and Oct1500 method were statistically significant (Z=-5.072,P<0.05).Conclusions Both methods can be used for Halcyon pretreatment plan QA.PD is superior to Oct1500 with respect to efficiency and spatial resolution-induced verification accuracy.The criteria of 2 mm/2% for PD,and Max Dose/3 mm/3% for Oct1500 was suggested.

ABSTRACT

Objective To construct a recombinant Bifidobacterium bifidum (Bb) vaccine of Pseudomonas aeruginosa,identify its protective effect in mice after immunization with the vaccine and challenged by Pseudomonas aeruginosa.Methods The OCR-amplified outer membrane protein OprH gene was cloned into vector pGEX-1λT to obtain the plasmid pGEX-OprH.The pGEX-OprH was then electroporated into Bb to obtain the rBb-pGEX-OprH vaccine.After PCR identification,the expression of rBb-pGEX-OprH was induced by Isopropylβ-D-Thiogalactoside (IPTG),and the expression products were analyzed by SDS-PAGE and characterized by Western blotting;21 BALB/c mice were subdivided by random number table into vaccine group,empty vector group and Bb group (7 mice in each group),and immunized by intragastrically administration with rBb-pGEX-OprH,rBb-pGEX-1λT or Bb,respectively.Mice were immunized once a day and three times a week for three consecutive weeks,then the mice were challenged intranasally with Pseudomonas aeruginosa at four weeks after the first immunization,once a day for three consecutive days.Two weeks after the challenge,the mice were sacrificed to detect their lung bacterial loads.Sera were collected before vaccination,four weeks after the first immunization,and two weeks after the challenge.The sera were analyzed by enzyme-linked immunosorbent assay (ELISA) to detect the level of IgG.Results OprH gene of 660 bp was amplified by PCR;PCR results showed that the rBb-pGEX-OprH was constructed successfully;SDS-PAGE confirmed that the recombinant vaccine expressed a relative molecular mass 47 × 103 fusion protein,which was consistent with the expected result;Western blot results showed that the expressed protein could be identified by sera in mice infected with Pseudomonas aeruginosa.Differences of lung bacterial colonies between the three groups were statistically significant (F =1 506.793,P < 0.05).The number of bacterial colonies in the vaccine group [(7.691 ± 0.069) lgCFU/g] was lower than that of the empty vector group [(8.855 ± 0.027) lgCFU/g] or the Bb group [(8.958 ± 0.037) lgCFU/g,P < 0.05].There were statistically significant differences in serum IgG levels between the 3 groups 4 weeks after the first immunization (F =77.216,P < 0.05),the IgG level of the vaccine group (0.078 ± 0.005) was significantly higher than that of the empty vector group (0.054 ± 0.004) and the Bb group (0.052 ± 0.004,P < 0.05).Conclusion The rBb-pGEX-OprH vaccine is constructed successfully,and it could induce certain protective effect in mice.

ABSTRACT

Objective To investigate the basic situation of ultrasound departments in secondary and tertiary public hospitals in Anhui Province.Methods Ultrasonic quality baseline survey forms were issued by the Ultrasonic Medical Quality Control Center of Anhui Province in November 2018,and the subjects of the survey were ultrasound departments of public secondary and tertiary hospitals in Anhui Province.The survey consisted of six parts,including the basic information of the head of ultrasound department,the number of staff,the area of ultrasound department,the number of ultrasonic diagnostic instrument rooms,the number of ultrasonic diagnostic instruments,and the number of beds actually opened in the hospital.Results In this survey,the results of 205 hospitals in 15 prefectures and cities in Anhui Province were received.Among them,there were 62 tertiary hospitals and 143 secondary hospitals.Most of the heads of ultrasound departments in Anhui public secondary and tertiary hospitals had a bachelor's degree,a middle or senior professional title,and a professional life of more than 20 years.The average number of ultrasound department staff was 9.3 in the secondary hospitals and 19.3 in the tertiary hospitals.The average area of the ultrasound department was 195.1m2 in the secondary hospitals and 505.9 m2 in the tertiary hospitals.The average number of ultrasonic diagnostic instrument rooms was 4.9 in the secondary hospitals and 11.3 in the tertiary hospitals.The average number of ultrasound instruments was 4.9 in the secondary hospitals and 11.5 in the tertiary hospitals.The average number of beds actually opened was 510.7 in the secondary hospitals and 1407.5 in the tertiary hospitals.Conclusions At present,ultrasound instruments in secondary hospitals in Anhui Province can basically meet the needs of clinical work,and there is a relative shortage of ultrasound instruments in tertiary hospitals.The shortage of staff in ultrasound department is a common problem faced by secondary and tertiary hospitals,especially tertiary hospitals.

ABSTRACT

Objective: To explore effect and mechanism of Jiaotaiwan (JTW) on cognitive impairment in diabetic mice. Method: The 40 db/db mice of spontaneous diabetes were randomly divided into model group, JTW 1 group (1.68 g·kg-1), JTW 2 group (3.36 g·kg-1), and JTW 3 group (8.40 g·kg-1), with 8 mice in each group, and 8 C57BL/6J mice were included into normal group. After 8 weeks of treatment, behavioral test,fasting blood glucose (FBG), fasting insulin (Fins), insulin resistance index (HOMA-IR), total triglyceride (TG), total cholesterol (TC), free fatty acids (FFA), high density lipoprotein (HDL), and low density lipoprotein (LDL) were detected. Western blot was used to detect protein expressions of pSer199, pSer202, pSer214, p-Akt, glycogen synthase kinase-3β(GSK-3β), phospho-GSK-3β(p-GSK-3β). Result: Compared with model group, FBG, Fins, HOMA-IR, TG, TC, HDL, FFA in JTW group decreased significantly (P0.01), with a significant improvement in cognitive function (P0.01), protein expressions of taupSer199, pSer202, pSer214 and GSK-3β in hippocampus of mice decreased significantly (PP0.01), and expressions of p-Akt and p-GSK3 beta significantly increased (P0.01). Conclusion: JTW 3 (Coptidis Rhizoma-Cinnamomi Cortex 10:1) can alleviate insulin resistance and cognitive impairment. The mechanism may be related to the inhibition of over-phosphorylation of tau protein, the down-regulation of GSK-3β protein expression and the up-regulation of p-Akt and p-GSK-3β expressions.
